Referring
Students
- Referral
to SDCP services is for children who have displayed significant and
prolonged social, emotional and behavioural problems in schools and
as a consequence have developed further inappropriate behaviours.
- Referral
should only be made after a range of interventions have been tried and
evaluated by the referring school.
- Referral
can be for either Outreach or
On Site services.
SDCP
programs are seen as short term interventions and not as continuing placement.
Placements are made for one term and on some occasions extended for a
second term.
At
all times students remain enrolled at their mainstream schools.

read carefully Section 6 of the Guidelines
document for essential information regarding the referral of students.

Download
Application forms and Guidelines - Read carefully.
- Called
a PSG for the student and attached the minutes of the PSG that indicate
placement for SDCP services is recommended.
- Parents
fully understand that onsite programs involve them in transporting their
child to and from the unit and that they are willing to do so.
- Application
forms X Y Z and Parent Permission are completed in full.
- Three
(3) copies of the application are to be mailed to arrive or be delivered
in person to the SDCP by 4.00 pm on the due date for each term.
